Quick Interest Form — Fill out to hear more about our team. It doesn't commit you to anything! Returning players can skip this one.
Player Info Form — Fill out as soon as possible if you're pretty interested in being on the team! Returning players, DO fill this out again!
Waiver & Release of Liability and Local Medical Authorization. You'll need a parent/guardian's signature (unless you're 18). Three options:
Option 1: DIGITAL Waiver & Med — Make sure you e-sign it (a link will be emailed to you).
Option 2: DOWNLOAD Waiver & Med — Download, fill out, and bring it to your first practice. (Or, fill it out and send a picture of it.)
Option 3: AT PRACTICE — You can also fill out copies we have on hand at your practice—with a parent/guardian if under 18.
Parkway Team Payment of $75. Venmo preferred (user id is timothy-dorsch, 314-518-7363, https://venmo.com/timothy-dorsch). Or, you can bring a check to practice (payable to Tim Dorsch).
Optional Team Donation — Please consider adding a "team donation." Our team can use extra funds for team and tournament scholarships, helpful equipment, etc. We try to keep required costs low so every extra dollar helps!
Order your uniform. You’ll order through the Breakmark site (link TBA). We’re getting NEW (and NICER) jerseys this year!
Varsity players will get a uniform bundle for ~$100. It will include a dark jersey, a light jersey, shorts, a practice reversible, and a five-panel hat.
JV players will have the option of getting the uniform bundle or a single dark jersey for ~$35.
The Girls team jersey plan is TBD.
Online USAU Fee & Online Waivers — By the first Saturday in March, create/go to your USAU account at https://play.usaultimate.org/members. Then:
a) Sign your online waivers on your USA Ultimate member's page and pay your membership online. See detailed instructions below. If you're a new player, make a note of your USAU ID #. Here are the membership options:
$20 — Select "Affiliate Player" (this covers MO state tournament participation).
$45 — Select "Youth Player" if you're confident that you'll also be going to the Youth Club Championship in Colorado in August. If you're unsure, you can purchase the "Affiliate Player" option and upgrade later if you go to YCC.
$0 — Select "Affiliate BIPOC" if your Member's Ethnicity Demographic qualifies (update via Account > #3).
b) AFTER you have done the previous step, fill out this 30-second form so that we are notified and can add you to our official USAU team rosters.
(BONUS) If you've done all of these things, hug someone because you're all set!

Detailed instructions to navigate the USA Ultimate site, register, and pay the USA Ultimate annual membership fee:
—If you’re a new player:
Sign up for an account on USAUltimate.com (click the Join USA Ultimate button).
As you register, you’ll fill out the necessary waivers.
You’ll be assigned a membership #.
After you’ve finished registering, you’ll need to pay for your membership:
Click on Manage Account (upper-right of page). Click either the “Purchase Membership” or the “Pay My Dues” button.
Select the correct membership option (see the options under #5 below) and proceed to payment.
—If you’re a returning player:
Log in to your account on USAUltimate.com.
Click on Manage Account (upper-right of page).
Click the buttons to sign your USAU waivers for the year.
Pay for your membership:
Click either the “Purchase Membership” or the “Pay My Dues” button.
Select the correct membership option (see the options under #5 below) and proceed to payment.
Email your membership number to tim.dorsch@gmail.com. Then, we can add you to our team’s roster.
